A PAR-Q must be completed before commencement of any exercise at JWC Health & Fitness.
Upon completion of this document, I agree that I have completed a physical activity readiness questionnaire and that I'm happy with the content. I know of no reason why I should not participate in any physical activity.
Should my health change or should I require advice or support with injuries or health conditions I will inform a JWC Manager.
I understand and am aware that participation in JWC Health & Fitness services involves risk. With the knowledge and understanding of these risks I choose to continue participation. I agree to indemnify JWC Health & Fitness Ltd against all claims arising out of participation by me in any physical exercise whilst under the supervision of my instructor.
All JWC memberships are non-refundable or transferable.
Membership may be suspended at the discretion of a JWC manager for periods of leave greater than 1 month. Written notice must be provided including a return date.
Recurring Monthly Payments
Monthly memberships at JWC Health & Fitness Ltd are on a recurring monthly basis for a minimum term of 3-months.
Your membership will automatically be extended once the Minimum Membership Period has ended for consecutive 1 month periods unless the agreement has been terminated or cancelled with notice. If you wish to cancel your membership please provide us with one months written notice, your membership will be terminated following a final payment after cancellation.
Membership may be suspended at the discretion of a JWC manager for 1 month increments, up to a maximum of 3 months per year, a 10% admin fee will be taken during months of suspension. Written notice must be provided including a return date.
All personal training payments must be made prior to sessions commencement unless other arrangements are made with JWC management. Pre-paid sessions are non-refundable.
Cancellations must be made with at least 24 hours notice; otherwise full payment will be charged.
Your personal training agreement is with JWC Health & Fitness Ltd and should your assigned coach no longer be available to carry out your pre-arranged sessions, we will try our upmost to rearrange this with an alternative coach with minimal disruption to your routine.
Should you have any grievance with your coach, any other JWC staff member or our facilities, then we ask that you speak to a member of management in the first instance who will attempt to resolve this issue quickly and efficiently.
JWC Health & Fitness will store my personal data, including but not limited to;

• Results Tracking Information
• Health Information
• Payment Details
We would never share this information with a third party without your consent.
JWC Health & Fitness regularly take photos and videos of sessions for promotional purposes, this content may be used across social media platforms, on our website, email marketing and printed literature.
Promotional Offers and Discounts
Promotions that include 'T&C's apply' will have restricted eligibility as they may be targeted towards specific membership types, returning members and/or non-members. This will vary between offers and is at the Managers discretion.
